Documents to bring to your child’s initial paediatric assessment

Please bring a referral letter from your referring doctor. A referral letter is a requirement for you to be able claim a Medicare rebate for a specialist consultation.
Your child’s Medicare card
Your child’s Blue Book/birth related medical records
Any relevant reports from previous assessments, allied health professionals (such as speech therapists, occupational therapists, physiotherapists and psychologists), school counsellor reports, any other specialist’s reports and blood/imaging test results
Your child’s school reports if your concerns relate to learning difficulties or ADHD symptoms
You may be requested to complete a questionnaire if your concerns relate to behaviours, learning difficulties, neurodevelopmental conditions (autism, ADHD, developmental delay) and toileting difficulties (bedwetting, daytime urinary incontinence, faecal incontinence, constipation). It is helpful if these are completed and returned prior to the day of the appointment.

Appointment Duration

Please note that depending on case complexity the initial consultation may take anywhere between 45- to 90 minutes. Please allow yourself sufficient time on the day.

Follow up consultations may range from 20-60 minutes in duration.

To help occupy them, your child may bring a toy of their own or a book if they choose to do so.
